You can schedule your free and convenient on-site screening now at one of over 50 CPS locations. On-site screenings are available between January 25, 2016 and March 18, 2016. PHYSICIAN BIOMETRIC SCREENING* FORM AVAILABLE FROM 1/6/2016 THROUGH 3/18/2016

You can go to your physician’s office to have your biometric screening completed. If you choose to have your biometric screening completed at your physician’s office, print the physician form to take with you to your physician’s office. Healthways will accept biometric screenings conducted in the last nine months (no results collected prior to 6/1/2015 will be accepted). Simply have your physician complete this form with the data that was collected at that time and fax to 615-349-9095 submit prior to the 3/18/2016 deadline. If your physician submits biometric results on any other type of form, the results cannot be accepted. You may not complete your physician’s office biometric screening on work time.

Retail health and urgent care clinics (e.g. Healthcare clinic at Walgreens, CVS Minute Clinic, or Target Clinic) may be an option for completing your screening and may be covered by your CPS health insurance if you are enrolled in the BlueCross BlueShield PPO or a UnitedHealthCare health plan. To locate an in-network facility please contact BCBS at 1-800-331-8032, www.bcbsil.com, or UHC at 1-800-905-4619, www.myuhc.com. Please note that there may be an out-of-pocket expense that you incur for completing the screening at a retail location.

OTHER IMPORTANT SCREENING REMINDERS

All wellness participants are expected to complete the screening to avoid the non-participation payroll deduction. If you have a medical reason for being exempted from the biometric screening, including pregnancy, you may request a Screening Exemption form from Healthways by calling 1-866-556-7671. Forms must be completed by your physician and returned to Healthways by 3/18/2016 in order to receive credit for Step 1; Complete a Biometric Screening.

All CPS employees and their spouses/domestic partners/civil union spouses enrolled in a CPS health plan that do not participate in the Chicago Lives Healthy Wellness Program will be subject up to a $600 per year non-participation deduction for each non-participant.

Condition Management is a comprehensive program that supports individuals with specific health conditions, such as diabetes, asthma, chronic obstructive pulmonary disease (COPD), coronary artery disease (CAD), and heart failure. This program is free of charge and completely confidential. It provides you with a personal health nurse available by phone or in-person at a time that’s convenient for you. Work with your medical provider to develop a personalized prevention plan, where your doctor develops a strategy with you to manage your health, including planning the preventive services and screenings you may need over the next 5 to 10 years. Report a medical visit with your physician and earn 24 participation points.
  • Complete a dental visit* from 1/1/2016 through 9/30/2016 and earn 12 points. Maintaining good oral health is important to your overall health as well. It’s recommended that most people get a dental visit every six months, but your dentist may recommend more frequent or fewer visits, depending on your dental health history.
