COMPLETE A WELL-BEING 5 ASSESSMENT (WB5) by February 28, 2017

Everyone enrolled in the Chicago Lives Healthy wellness program MUST complete a WB5. You have until February 28, 2017 to complete your online WB5 for the 2017 plan year.

The deadline to complete the Well-Being 5 Assessment was extended through Thursday, March 2, 2017.

The Well-Being 5 Assessment (WB5) measures your current well-being and is used to help you and your Health Advisor identify potential areas of focus for your personal 2017 Wellness Program goals.

The WB5 is a simple, confidential questionnaire that looks at your lifestyle habits and current health status to see what may be helping or hurting your overall well-being. You will answer a few focused questions and get instant feedback in your personal Well-Being Report, including suggestions on ways to increase your well-being.

All questions are simple to understand and easy to answer. It will take 15 minutes to complete the questionnaire.

1. RETURNING USERS: Log on to Well-Being Connect (WBC), using your registered username and password. You will immediately be prompted to complete the WB5. If your biometric screening results are not present, please provide as many lab test results and biometric information as you can and continue to the next page. Upon completion of the WB5, you will immediately receive your well-being report.

2. NEW USERS: Go to WBC and click the “Sign Up” button. You will be prompted to register and create a username and password. Please document your username and password as you will need this information to access Well-Being Connect in the future.

You will then be prompted to complete the WB5. Once you begin the WB5 please provide as many lab test results and as much biometric screening information as you can and then continue to the next page. When you click the Finish button, your assessment is complete. Within 24 hours your WB5 results will be processed and you will receive an email notifying you the results are available for your review. You can then create your Well-Being Plan to help you set and achieve your health goals.

When your actual biometric screening data is available and you have created your Well-Being Plan, you will be notified via email and you can review your new Well-Being Report.

Do you need help completing your Well-Being 5 Assessment (WB5)?

Health coaches in the Chicago Lives Healthy wellness program are available to help you complete your Well-Being 5 Assessment. Here are two ways to sign up for a Well-Being Help Session:

2. Call Healthways at 1-866-556-7671 and sign-up for a Well-Being Help Session through a customer service representative.

WHAT IF I DON’T COMPLETE THE WBA?

If you or your covered spouse, domestic partner or civil union spouse do not complete the WB5 by February 28, 2017, the employee will incur a $50 per non-participant increase in monthly health care contributions for each individual who does not complete the WB5.

A Healthways' Health Advisor will call you when they have the results from your biometric screening and your WB5 on file for 2017. Health Advisor calls can be completed from January 5, 2017 through March 31, 2017.

WHAT IS THE HEALTH ADVISOR CHECK-IN CALL?

The Health Advisor Check-In Call is an outbound call to you from one of Healthways' certified health professionals. During this 15 minute call, the Health Advisor will review your biometric screening and your Well-Being 5 Assessment results (WB5) with you. The Health Advisor will talk with you about your health and well-being and if you wish, develop realistic health or lifestyle improvement goals with you.

Please update your primary phone number in Well-Being Connect so that the Healthways Health Coach can call you to complete the Health Advisor Check-In Call.

IMPORTANT INFORMATION ABOUT YOUR HEALTH ADVISOR CHECK-IN CALL:

1. Individual Health Advisor Check-In calls will be made to each wellness participant.

2. For those of you that were not required to get a biometric screening in 2017, your prior biometric screening results and your 2017 Well-Being 5 Assessment (WB5) results will be discussed when you complete your Health Advisor Check-In Call. Ongoing Participation Paths

Beginning with the 2017 plan year, ongoing participation will change. For some participants, ongoing participation is REQUIRED and for others ongoing participation is OPTIONAL. The Chicago Lives Healthy wellness program no longer requires any participant to earn quarterly participation points; however, participants who are required to be in a Health Improvement Program (HIP) must complete required activities on a quarterly basis.

Health Improvement Programs: Here is a brief listing of the Health Improvement Programs that are offered to those persons whose quarterly participation is REQUIRED in 2017. METABOLIC SYNDROME COACHING PROGRAM

Telligen is a long term partner with the City of Chicago and is dedicated to the improvement of member’s quality of life through relationship based health coaching. The goal is to assist the member with improving current or potential medical conditions to live a healthy, active life. The dedicated health coach will assist the member with development of a personal health improvement action plan tailored to their unique risk factors and provide strategies, education and support to the member while making progress toward improving their health.

Participants in this program will learn to manage their health risk factors and make progress towards improved biometric numbers and/or lowering the number of metabolic risks. Telligen is managing this telephonic coaching program and their health professionals will be in contact with you by mail to tell you more about the program and begin the enrollment process.

CONDITION MANAGEMENT PROGRAM

The Telligen health professional will coordinate the participant’s healthcare treatments, engage medical providers on evidence-based practices and educate and encourage the participant’s self-management skills regarding their health conditions.

TAKING CONTROL OF YOUR HEALTH (TCOYH)

Since 2009, The Illinois Pharmacists Association (IPhA) and the Midwest Business Group on Health (MBGH) have partnered to provide employers and their covered employees with the chronic disease management program, Taking Control of Your Health (TCOYH).

The program is designed to increase your knowledge and understanding of your condition while learning about proper diet, exercise and stress management techniques to adopt healthy behaviors to manage and improve your diabetes. Participants in this program will have private, face to face counseling sessions with a personal diabetes coach, a specially trained pharmacist who works closely with the participant and their physician, to learn how to better manage and monitor their chronic conditions (diabetes, high blood pressure, cholesterol), and support the effective use of their medications.

INNERGY

Offered by Healthways in collaboration with Johns Hopkins Medicine, Innergy® is a hightouch, personalized solution proven to sustain weight loss. Success is defined as not only initial weight loss but sustained weight loss over a two-year period. In this way, it overcomes the challenges of programs that can perpetuate unhealthy lose-gain weight cycling by focusing on rapid initial weight loss alone and not building critical weight loss maintenance skills and confidence.

Participants in this program will have an Innergy coach. The Innergy coach will help you make great choices today and will support you in building skills and strategies to achieve and maintain a healthier weight for a lifetime. Healthways is the vendor managing this coaching program and their health professionals will be in contact with you regularly by mail to tell you more about the program and confirm your enrollment.

How were participants selected for REQUIRED Health Improvement Program (HIP) participation?

Based on a combination of factors including biometric screening data and, for PPO participants, medical claims data, participants were assigned to a Health Improvement Program (HIP). Participants who met one of the following sets of criteria were selected for REQUIRED participation:

• Participants who have 4 or 5 markers for Metabolic Syndrome. Metabolic Syndrome is the name for a group of risk factors that raises your risk for heart disease and other health problems, such as diabetes and stroke. You can have any one of these risk factors by itself, but they tend to occur together. Participants who have four or five metabolic risk factors or markers will be included in this category. In accordance with the National Institutes of Health guidelines, the risk factors or markers for Metabolic Syndrome include:

> High triglyceride level — 150 milligrams per deciliter (mg/dL), or 1.7 millimoles per liter (mmol/L), or higher of this type of fat found in blood
> Reduced high-density lipoprotein (HDL) cholesterol — less than 40 mg/dL (1.04 mmol/L) of this “good” cholesterol
> Increased blood pressure — 130/85 millimeters of mercury (mm Hg) or higher
> Elevated fasting blood sugar — 100 mg/dL (5.6 mmol/L) or higher
> Large waist circumference — a waistline that measures at least 35 inches (89 centimeters) for women and 40 inches (102 centimeters) for men

• Participants who are identified as likely to need significant healthcare resources or have worsening of an existing health condition during the coming year.
Predictive modeling software was used to identify those participants in higher risk categories. Condition Management services can help these participants by coordinating healthcare with their doctors, and educating the participant on managing his or her conditions. Some of the conditions requiring care management or disease management services are:

> Cardiac
> COPD
> Diabetes
> Chronic Kidney Disease
> Asthma
> Depression & Obesity (interwoven into all programs as applicable)
> Multiple chronic conditions

How are participants notified to enroll in the REQUIRED Health Improvement Program (HIP)?

If you are REQUIRED to participate in a Health Improvement Program you received information regarding your (HIP) assignment in the Chicago Lives Healthy wellness program launch letter dated January 2, 2017 from the Chicago Benefits Office. The Health Improvement Program (HIP) vendors Telligen, TCOYH and Healthways are communicating by sending written correspondence and making telephone calls encouraging REQUIRED participants to enroll by April 30, 2017.

What is the deadline for initial enrollment in my REQUIRED Health Improvement Program (HIP)?

Initial enrollment for all Health Improvement Programs offered by Telligen, TCOYH or Healthways must be completed by April 30, 2017.

Where can I go to determine if I have completed my REQUIRED active quarterly participation?

If you have been assigned to a Health Improvement Program (HIP) the quarterly point requirement has been replaced by an “active participation” requirement. The active participation requirement will differ by program. Following your initial enrollment session, individualized coaching sessions will be agreed upon and communicated to you for each quarter by your health coach.

The quarterly active participation dates are:

April 1, 2017 to June 30, 2017
July 1, 2017 to September 30, 2017
October 1, 2017 to December 31, 2017

When you have met the program goals agreed upon for each quarter, a “green star” will appear on the rewards center tab, in the “My Progress” column next to, “Complete Quarter 2 Requirement”, in the HEALTHWAYS Well-Being Connect (WBC) portal. Here is an example of what your rewards center will look like if you have completed the Q2 requirement:

What if I choose not to participate in my REQUIRED Health Improvement Program (HIP)?

If you choose not to participate in your REQUIRED Health Improvement Program, the City of Chicago employee will incur a $50 per non-participant increase in his/her monthly health care contribution.

Participation is Optional: If you have been assigned to the “optional participation” requirement, you may continue to use the Well-Being Connect portal, participate in on-site education sessions, or work with a Healthways’ Health Coach. The Chicago Lives Healthy wellness program will no longer require participants to earn quarterly participation points.

The online portal will help you live more fully by getting to know you better, helping you set goals and connecting you to support to achieve your goals. Onsite Health Education Sessions

One of the best parts of the Chicago Lives Healthy wellness program is access to free, top-quality health education sessions for participants. Whether you’re setting goals to lose weight, managing an ongoing lifestyle issues like quitting smoking or just want to learn more about the Chicago Lives Healthy wellness program, a health education session allows you to focus specifically on your questions, needs and goals with a health coach.

ACCESS ONSITE HEALTH EDUCATION SESSIONS

Voluntary Telephonic Coaching Sessions

Eligibility for telephonic coaching with Healthways is determined by a participant’s biometric screening and Well-Being 5 Assessment results. Telephonic health coaching is a voluntary program that provides personal support from a health coach to help individuals with moderate to high health risk factors overcome the challenges that stand in the way of their good health. Telephonic coaching with Healthways is not available to people who are REQUIRED to participate in a Health Improvement Program (HIP) program.

How participants were notified that their ongoing participation is OPTIONAL?

If ongoing participation is OPTIONAL for you for the 2017 wellness program year, this information was communicated to you in the Chicago Lives Healthy wellness program launch letter dated January 2, 2017 sent by the Chicago Benefits Office. If you are unable to locate that letter and you are uncertain about your ongoing participation status for the 2017 program year, you can also confirm your status by:

1) Confirming that there is a green star in the “My Progress” column in the rewards center tab, next to the “Voluntary Ongoing Participation” section. See screenshot below showing what it looks like; or,

2) Call Healthways at 1-866-556-7671 to confirm your ongoing participation status.

What tools are available to me in the Chicago Lives Healthy wellness program if my ongoing participation is OPTIONAL?

You may continue to use the Well-Being Connect portal, participate in on-site education sessions, or work with a Healthways’ Health Coach. If you are enrolled in the Blue Choice Opt PPO Plan you may also be eligible to participate in a disease or care management program offered through the health plan.
